RECALL: Ushio Indiglow LED T8s due to injury hazard

July 22, 2020 – Ushio America is voluntarily recalling Indiglow LED T8 black light lamps (Item #3000677) with a 4-ft tube because they can overheat, causing the glass tube to fall and strike those standing nearby.

The lamps are direct drop-in replacements for 30W and 32W T8 fluorescents. The name USHIO and brand name Indiglow are on the lamp. The manufacturing date code is imprinted on the silver aluminum end cap of the lamp, indicated by 16xx or 17xx.

As of July 2, 2020, the company has received one reported incident in Canada of a lamp falling, and no injuries.

About 75 affected units were sold in Canada between June 2017 and February 2018. They were manufactured in China.

If you are affected by this recall, stop using the product and contact Ushio America for instructions on how to return the product to receive a full store credit or a free replacement lamp.
